Ak-Chin Diabetes Prevention Physical Activity Program
The physical activity program consists of 1-on-1 nutrition counseling and or fitness training at our fitness facility or in the home. Small group training at our fitness facility. Physical activity and nutrition education weekly with the preschool. Elders group fitness classes 2 times per week as well as elders Olympic games once per year at Gila River Indian Community. Group Exercise classes 2 times per week that are open to the community. Summer youth fitness program 2 times per week, and several community walks per year promoting different community events such as Ak-Chin Diabetes Awareness Day, Susan G. Komen Breast Cancer Walk, Native American Recognition Day, JDRF walk, etc.

Program Description

Program Purpose
The purpose of the physical activity program is to promote physical activity options and Diabetes Awareness as well as prevent diabetes and related health issues.

Program Key Features
The key features of how our physical activity program is carried out is started by provider referral, family/friend referral, self referral, Ak-chin Health Clinic referral/consultation. Then schedule office visit. From there we provide an orientation as far as defining client goals/needs, provide fitness orientation packet, DM packet or Pre DM packet. Make recommendation for type of intervention. If enrolled in program there are 3 avenues of intervention - Nutrition Counseling, Home Visit (exercise prescription/counseling), Fitness Center Training (1-on-1 or small group training).

Most Excited About
What we are most proud of is the impact we are starting to have with specific individuals, and beginning to change their life. We are most excited about the excitement starting to build within the community and how some of the community are starved for the education and help and how we are able to provide that void.
