Program Purpose:
To provide community awareness in the area of the prevention and management of diabetes. Also to improve the health status of our community members by encouraging physical activity and healthy lifestyles.

Program Key Features:
Diabetes prevention program, Diabetes management program, blood glucose and blood pressure screening, pysical activity and exercise, health education, community presentations and workshops.

Most Excited About:
The new fitness facility has increased the rate of participation of community members.

Various Tribal Departments, Arizona Western College, University of Arizona Extension, La Paz County.
